Color Designs for the Winter Season
This includes the basketball and cheering designs which you can have put on your apparel. The CODES are also available on this page. Please put the code of your design in the box next to your choice when ordering.

Screen Print Order Form
This form is for all the designs except the embroidery design. Please remember to include the code of your design which is found of the color design page.
Embroidery Order Form
This form is only for the embroidery design which can be found on the color design page.
Steps to Ordering
1. Decide if you would like screen printing or embroidery done on the item. (All the designs are screen print but one)
2. Print out the order form.
3. Look at the color designs page and decide which design you would prefer.
4. Write that design code in the box on the order form next to the item you will be ordering.
5. Total up your order. Be sure to include any personalization that you might want.
6. Make checks payable to: St. Francis of Assisi Schools
7. Bring the order form and check to the Cabrini school office.
When orders are completed, they will be brought to Cabrini and distributed to the students to take home. If you are at another school, we will make sure your coach would get the item to hand out.
